Insurance co-payments will be due on the day of the evaluation
For insurance plans for which Dr. Barclay is out-of-network, payment will be due at the time of scheduling the evaluation. If requested, she can complete the necessary paperwork that you will need to submit for insurance reimbursement (e.g., superbill).
